Cape Town’s harsh winter weather, marked by snow on Table Mountain and fierce winds, hasn’t deterred locals from seeking free medical care aboard a Chinese ship docked in the city’s harbor.
As South Africa grapples with a financial crisis that has strained public services, many residents find themselves unable to afford private healthcare due to skyrocketing costs.
In response, the arrival of China’s Peace Ark last week has offered a lifeline to more than 2,000 South Africans, who have received treatments ranging from maternity check-ups and cataract surgeries to traditional cupping therapy.
The Peace Ark, part of a joint exercise between the South African and Chinese militaries, can treat up to 700 patients daily and houses a 100-member crew, 300 beds, 20 intensive care units, operating theaters, clinical departments, and even a rescue helicopter.
For some, this medical ship represents a rare opportunity to receive healthcare that is otherwise inaccessible. Lucy Mnyani, a Cape Town resident, shared her joy with local media after seeing images of her unborn child for the first time. “I had been going to day hospitals in Gugulethu and Langa, but they never sent me for a CT scan,” she said.
Others echoed similar sentiments. Joseph Williams, who waited in line for treatment, expressed his relief to the SABC: “When you go to the local clinic, you sit for hours before they help you. Here, the service was very quick, so I’m grateful I came.”
Public Services in Crisis
The influx of patients to the Peace Ark highlights a growing concern in South Africa: the country’s public health system is struggling to meet the needs of its people. With a backlog of 80,000 patients awaiting surgeries in the Western Cape province alone, the situation is dire. The Peace Ark has performed 57 surgeries so far, a small dent in the overwhelming demand.
“This is in Western Cape, which arguably has one of the best health systems in the country,” noted Dr. Saadiq Kariem, head of the Western Cape Health Department.
He added that the Peace Ark had extended its services to the homeless and elderly, who often lack access to medical care.
Dr. Kariem, who also received a check-up aboard the ship, remarked, “From registering to completing my care took me an hour. It’s something that would take much longer at our public healthcare facilities because you have many more people requiring services.”
National Health Insurance Sparks Debate
The timing of the Peace Ark’s visit coincides with South Africa’s ongoing debate over the National Health Insurance (NHI) scheme, a controversial new health policy that aims to provide universal healthcare.
Despite facing legal challenges and opposition from private health companies, the ruling African National Congress (ANC) is determined to push forward with the NHI.
Critics argue that the scheme could lead to a mass exodus of healthcare professionals seeking better opportunities abroad, further crippling the country’s healthcare system.
Dr. Shuaib Manjra, chairperson of the Health Justice Initiative, emphasized the challenges facing the public health sector.
“It shows the public health system in the province and in the country is not serving the people as it should. Often you find people spending an entire day at a clinic waiting to be seen. There are major backlogs at hospitals, budgets and posts are being cut, and often this results in people missing out on up to two days of work after waiting to be seen for a simple procedure.”

With only 14% of the population covered by private medical care, the remaining 86% of South Africans rely on overburdened state clinics and hospitals.
The Peace Ark’s popularity underscores the urgent need for effective healthcare solutions in a nation where access to medical services remains a significant challenge.
